Little Relief for Asthmatics

We usually expect environmental
regulations to make us healthier in the
long run. Well, there’s one coming down
that some people aren’t so sure about.
Reporter Shawn Allee says it has to do
with propellants in asthma medicine:

Maureen Damitz struggles with asthma.

She’s got it and two of her kids do, too.

But fighting it is also a career.

Damitz is with the Respiratory Health Association of Metropolitan Chicago – it’s the
nerve center for asthma issues in her area.

She says recently, pharmacies have been running out of a familiar asthma inhaler.

“Our phones started ringing off the walls months ago. All of a sudden patients
started getting these new inhalers, and no one was prepared for that.”

The old-standby has been the albuterol inhaler – it’s for quick relief.

Damitz says there is a cheap generic, but it’s got a propellant with Chloro-fluoro-carbons
or CFCs.

And, the government’s banning CFC albuterol inhalers.

Damitz says some patients will miss them.

“When you’re spraying it, it comes out with quite a blast.”

“People mistake that as, ‘it forces it into my lungs’; it doesn’t, it’s just the type of
propellant.”

Three new inhalers have the same medicine but a different propellant, known as HFA.

“The new HFA comes out much softer and its warmer when it comes out. They
mistake that as, ‘Oh, my medication doesn’t work.’”

Damitz says studies show the new inhalers work just as well or better than old ones, but
some patients report just the opposite.

Regardless, no one will have a choice soon. By January, no pharmacy can sell albuterol
inhalers with CFC propellents.

Why?

“Originally it arose from the concern that CFC’s were damaging the atmosphere.”

Dr. Nicholas Gross is an asthma specialist.

He says CFCs used to be in many things – refrigerators, air conditioners, and asthma
inhalers.

But CFCs deplete the ozone layer in the upper atmosphere. That lets more solar radiation
through and causes skin cancer.

The government banned CFCs in most products.

But drug companies got exemptions and were slow to develop alternative propellants.

In 2005, the government asked a medical panel to speed things up.

“They were concerned nothing much was changing. It looked like companies were
going to keep claiming exemptions indefinitely, so they asked what we would
recommend they should do about that.”

Gross and other panelists found three competing albuterol inhalers with new propellents.

So, they recommended a ban start next year. Now, Dr. Gross regrets that ban.

“One thing I don’t think anybody paid enough attention to was the fact that it was
going to be much more expensive in the HFA version than the CFC version.”

CFC-based albuterol inhalers cost about thirteen bucks a pop.

New HFA ones cost three times that.

There won’t be a generic inhaler with the new propellant until 2010.

Dr. Gross worries some patients will go without.

“I think it’s very difficult for the FDA to turn around and rescind itself. It means
somebody made a mistake and in government that’s not something you’re allowed
to admit.”

But, the FDA is sticking with the ban.

One asthma expert is more at ease with the transition.

He’s Paul Greenberger – head of the Academy of Allergy, Asthma and Immunology.

He says if patients puff through new, expensive albuterol inhalers quickly – there might
be something wrong with how they’re using them.

“We don’t want people using them everyday if they can help it. We have to take a
look at their overall asthma control – do they need better therapy, frankly than
these albuterol inhalers?”

Of course, that might mean a doctor’s visit and new meds.

Dr. Greenberger says all of this is expensive, but he still supports a ban on CFC albuterol
inhalers.

He says if patients get treatment that’s also better for the atmosphere, well, that’s
priceless.

people got sick and more than 100 died
from contaminated drinking water. It’s
still the biggest outbreak of waterborne
disease ever in the United States. It
happened because a parasite got into the
water supply in Milwaukee. Since then,
there have been major changes in water
systems across the nation. Erin Toner
reports:

Fifteen years ago, 400,000

still the biggest outbreak of waterborne
disease ever in the United States. It
happened because a parasite got into the
water supply in Milwaukee. Since then,
there have been major changes in water
systems across the nation. Erin Toner
reports:

Dr. Ian Gilson has been treating AIDS patients for 25 years. He says in all that time,
nothing’s been as bad as 1993 when Milwaukee’s drinking water was contaminated with
a parasite called cryptosporidium.

“We began to get reports of some of our patients having diarrhea that didn’t stop and we
had patients with weird stuff like an ulcer that was not related to acid, severe gall bladder
disease without stones. Ultimately by the time it was called a waterborne epidemic we
knew we had a big problem on our hands.”

Healthy people who drank the water, or brushed their teeth with it, or ate food that was
washed in it, had severe vomiting and diarrhea. But people with weak immune systems,
like those HIV with AIDS, couldn’t fight the parasite. And there weren’t good AIDS
drugs back then, so the patients just deteriorated.

“I distinctly remember several patients saying if you can’t get me over this let’s just be
done with this. One guy who was suffering terribly, we couldn’t seem to get him enough
morphine. And I ordered what I thought was a fatal dose of morphine because I thought
that was the only thing that was going to help him. And it actually relieved his pain.”

When it was all over, cryptosporidium killed 103 people with HIV and AIDS. Even after
15 years, the source of the parasite is still a mystery.

“The cause is not known and may never be known. There does not seem to be any
obvious explanation.”

Carrie Lewis is superintendent of Milwaukee Water Works. She says at the time of the
outbreak, the city pumped in water through an intake pipe about a mile off shore in Lake
Michigan.

The prevailing theory is that sewage overflows contaminated water in the bay, and that
the water was pushed toward the intake pipe and entered the treatment plant.

Lewis doesn’t buy it.

She says if human sewage was the source, people would have had to be sick to excrete
the parasite, and there’s no evidence of that. Some also speculate that cow manure
contaminated area rivers, but Lewis says regular testing in the watershed rarely finds
traces of cryptosporidium.

Lewis says she has no clue what happened, and she’s OK with that. She says what’s
important is what’s changed since then. Lewis says water testing at the time of the
outbreak amounted to taking a couple of samples a day – and that was considered good.

“Today we have hundreds of instruments testing the water every single second for all
sorts of different parameters, so the 15 years that’s gone by it’s a lifetime.”

The cryptosporidium outbreak so damaged Milwaukee’s psyche that people were willing
to do just about anything to make the water safe again. The city spent $90 million to
extend the intake pipe farther out in Lake Michigan. The filters at purification plants were
updated. And now the water is treated with ozone, which kills cryptosporidium.

What happened in Milwaukee caused changes around the country.

New federal regulations required water systems to test for the parasite and safeguard
against it. A drug was licensed to treat the disease.

Michael Beach is associate director for healthy water at the Centers for Disease Control
and Prevention. He says today people should have a lot of confidence when they turn on
their tap.

“Those types of outbreaks have virtually disappeared from the tracking system.”

Many water experts say municipal drinking water in this country is now the safest in the
world. They say the legacy of the Milwaukee outbreak is that water utilities are no longer
just managing a system of pipes and water mains – they’re in the business of protecting
public health.

Keri Hornbuckle says not much is known about PCB-11.

That’s because no one really expected to stumble on it.

“This finding is important, and I think it’s alarming. We should find out what the
health effects are for this chemical.”

Hornbuckle is a University of Iowa professor.

She looked at air samples from all over Chicago, and found the chemical in about 90% of
the samples.

She says the source is a mystery, though it might come from paints breaking down.

“Now, it could be that in all those places where we would expect to see the other
PCBs, we’d also see this one. But it’s a surprise, because no one thought it would be
produced.”

Hornbuckle says she found PCB-11 using a new, detailed way of analyzing air samples.

Other PCBs have been linked to cancer and neurological problems.

It’s one of those perfect beach days. Not too hot, not too crowded. Everything’s
just right.

Unless you don’t like rotting fish and dead birds full of maggots.

Hunter Brower is hanging out at this Lake Michigan beach. He says he’s seen a
lot of dead gulls here in recent years.

“It’s disgusting. We’re out here to enjoy our time and it’s just not right.”

The birds and fish are being killed by Type E botulism. Basically, it’s food
poisoning. For about a decade now, botulism has been killing huge numbers of
birds in the Great Lakes.

We’re talking about more than 75,000 birds – and scientists think that’s probably
a very conservative estimate. That’s because birds could be dying and not
reaching the shore. And it’s very hard to know how many fish are getting killed.
They’re harder to diagnose.

“It’s really one of nature’s most potent toxins.”

Mark Breederland is with Michigan SeaGrant. He says some studies show the
toxin can paralyze fish.

“They can actually lose their orientation and be bobbing up and down vertically
and that would be easy pickin’s if you’re a loon.”

Loons and other birds eat those poisoned fish, or, even grosser, they’ll eat the
maggots in dead birds on the beach, and get sick. The toxin can make birds lose
control of their neck muscles. Their heads fall in the water and they drown.

Beaches full of dead fish and birds aren’t great for tourism. But scientists are
more worried about what this means for endangered species – from the giant lake
sturgeon to the tiny piping plover.

Mark Breederland says they’re also worried about the thousands of migratory
birds that get killed on their way south in the fall.

“They’re just driving down their migratory highway, pulling over for a rest stop to
get something to eat, and that’s their last and final resting spot.”

So scientists are trying to pin down what’s going on.

There’s one main hypothesis. It involves some nasty little critters: invasive zebra
mussels and their cousins, the quaggas. They got into the lakes in the ballast
water of foreign ships.

Both mussels suck in lake water and filter it. They’ve made the lakes a lot clearer
than they used to be.

The clearer water means more sunlight can reach the lake bottom, and that
kick-starts algae growth. When the algae die, it sucks oxygen out of the water.
And, all of that is perfect for a bacterium that produces the botulism toxin to go forth
and multiply.

Okay, now, remember those pesky mussels? Scientists suspect they can take in
the toxin but they’re not affected by it. But fish that eat the mussels get sick.

One fish in particular loves to eat mussels. It’s the invasive round goby. And
there are lots and lots of gobies in the Lakes. That could mean lots of poisoned
snacks for bigger fish and birds.

Researchers have a bad feeling about all this, and they’re trying to confirm their
hunches.

Brenda Moraska LaFrancois is headed out on Lake Michigan to investigate. She’s
part of a team that’s collecting samples from the lake bottom.

She says this is a tough mystery to unravel.

“These are really complicated systems and unfortunately they’re continuing to
change.”

As soon as scientists think they have a handle on what’s going on, some new
invader gets in and messes everything up again. So it’s really hard to know what
could be done to stop the outbreaks.

The experts say, if you go to the beach, it’s safe to swim. But you shouldn’t eat any
fish or waterfowl that seem sick.

Your local wildlife managers might tell you: don’t touch it, but get something to
bury the dead animal down in the sand, so other birds won’t feed on it and spread the
toxin.

It’s called Viral Hemorrhagic Septicemia. Humans can’t catch it, but it
causes internal bleeding in fish.

The virus is hurting the region’s multi-billion-dollar sport fishing industry.

Now, researchers are finding that adding iodine – a common practice in fish
hatcheries – could prevent the virus from spreading.

Steve LePan is a biologist for the state of New York. He says a study at
Cornell University found Walleye eggs treated with an iodine solution were
not infected with VHS.

“We can’t say for sure that it’s exclusively the iodine that kills it. There may
be other things we do to the eggs that also affect the virus, as well.”

Those ‘other’ treatments include bathing Walleye eggs in Tannic Acid for a
few minutes before incubation.

LePan says there’s still a lot to learn about VHS, but he’s cautiously
optimistic that hatcheries can breed fish uninfected by the disease.

For six long years, the tiny metallic-green emerald ash borer has been a killing machine,
starting with millions of ash trees in Michigan and Canada, and then munching its way
into 10 states.

It was recently discovered in Missouri, and now, it’s in Wisconsin.

The prognosis is not good.

Darrell Zastrow is with the Wisconsin Department of Natural Resources.

“Our forests are not typically resilient against non-native species and that is true for
the emerald ash borer. It is generally considered to be a poster child for invasive
species.”

Officials in Wisconsin are doing what everyone else has done – restricting the movement
of firewood and telling people how to protect their trees.

Some promising treatments to fight the emerald ash borer are being tested, but so far,
nothing has worked at keeping the insect from spreading west.

Malaria kills more than one million people every year. The disease is
spread by mosquitoes.

Marcelo Jacobs-Lorena is a researcher at Johns Hopkins Malaria
Research Institute. He’s part of a team that has bred a genetically
modified mosquito. It can’t get infected by the malaria parasite.

“If we go to a region where malaria is prevalent and we are able to
substitute the local mosquito population by the modified mosquito that
cannot carry the parasite, then the net effect will be interruption of
transmission.”

Jacobs-Lorena says they’re confident the super mosquito is safe to
release into the wild.

But he says they’ll have to convince a lot of people that it’s okay to let it
out of the lab.

It’s summertime – sometimes known as

has the buzz on bug sprays:

Hot summer nights haven’t changed all that much since
1957. Sure, they’re selling sushi at ball parks these days.

But, for the most part, people are still using the same bug
spray.

DEET was designed in the 1950s for military use.
Stores started selling it soon after. At this kids’ baseball
game in northeast Ohio, there are as many opinions about
DEET and bug sprays as there are people.

Person 1: “I if I did use bug spray, I would use DEET. I heard that’s the
best.”

Person 2: “No, actually, I am concerned about any kind of chemical. I
don’t like to use it. I don’t think it’s necessary.”

Person 3: “I’ve been scratching all week.”

Julie Grant: “Why don’t you use it?”

Person 3: “Stinks.”

Person 4: “Probably looking for something without DEET, of course,
which is pretty much outlawed anyway.”

Child: “I have a bug bite on my neck.”


Person 4: “Yes, you do have a bug bite on your neck, which is why
we need to go look for something.”


Actually, DEET is not even close to being outlawed. It’s used
hundreds of millions of times each year – by backyard
barbeque-ers and soldiers, alike.

In all these years, there have been only fifty reports of skin
problems or seizures – and those were attributed to gross
overuse, and couldn’t be definitely connected to DEET.

But it is a plasticizer. If it touches your watch face, it will
smear the plastic. And that makes people a little nervous.

And while it does drive away a wide array of bugs, including
mosquitoes that carry West Nile Virus, DEET does not protect
against mosquitoes that carry malaria, or ticks that carry
lyme disease and rocky mountain spotted fever.

The worst thing about DEET for lots of people is that
stickiness and that evil smell.

Trisha Branden is a health editor with Consumer Reports
magazine. She says lots of people just don’t like using
DEET.

“But I think everyone has their own sensitivity, particularly
when it comes to the smell and feel of DEET. It’s better to
have some protection than none at all.”

Consumer Reports has surveyed bug sprays and crèmes on
the market.

They found that the higher the concentration of
DEET, the longer it protects you from bugs. At the highest
level, 98% DEET, it lasted half a day.
But some products contain as little as 7% DEET because it
smells so bad.

Some people want something else. The EPA recently
approved a new chemical – called Picaridin. It smells better.
Consumer Reports found that Picaridin products kept
mosquitoes away for up to four hours. That’s not too bad.

It’s better than the natural products. If you use soybean or
peppermint oil, Branden says you’ll probably have to reapply
a lot – at least every two hours.

“If you compare that to Picaridin product or the low DEET
products, that is not nearly the protection.”

Chemist Ulrich Burnier is on a team researching the next
generation of bug sprays. He believes there are more
effective chemicals out there. They’ve tested thousands of
chemicals and narrowed it down to seven.

“If we’re successful, the ideal product will give you 24 hours
protection, so you only have to apply once a day, you’ll be
protected around the clock. It’s not a plasticizer. And,
actually, this morning, I spent time trying to pick up odors
from these seven compounds, and none of us participating in this
study could detect any disagreeable odors from any of these
compounds.”

DEET still works pretty well. But mosquitoes and other
invading insects are thriving. As we experience climate
change, levels of mosquito-borne diseases like malaria are
on the rise.

When you’re at your nephew’s baseball game, and fending
off bugs, you might need to be a little patient. It could take a
few years. But experts say you should be able to find new
products that you like and that work for you.

A disease caused by something called chytrid fungus is sweeping through frogs. When the disease moves through a frog population it can wipe out 80% of the entire population. Scientists have been rushing to find something that might help.

Reid Harris is a biologist at James Madison University. He says he’s discovered there are friendly bacteria that live on some types of frogs. And they can kill the fungus.

“It does seem like the pathogen moves in this predictable wave, so you might be able to get out in front of that wave sort of like a fire line.”

Harris says it might be possible to give wild frogs extra doses of the bacteria to fight off the fungus. But first they have to make sure there won’t be side effects.

wildlife diseases that threaten animals and
people. Chuck Quirmbach reports:

Diseases such as West Nile Virus, Chronic Wasting Disease, Avian Flu, and
others are now often in the news. A website partly developed by the US
Geological Survey aims to track reports of the disease outbreaks around the
world.

Veterinarian Josh Dein leads the project. He says he hopes both health care
professionals and the general public use the online map.

“One of the things you can do is say ‘what’s happening in my neighborhood,
what’s happening in my state, my country?’ And maybe I’m traveling to
someplace else. Maybe I want to look to see what’s happening somewhere else.”

Dein says the map also allows people to narrow their search to specific countries, types of
disease and affected species.
